How to apply

There are 3 steps in taking medical leave:

1. Apply Online.

2. Submit a Medical Certification after you apply.

  • Must be provided in 15 calendar days from the date HR notifies you of your eligibility.
  • Certification is required for pregnancy disability, but is not required for the birth or to care for a newborn.
  • To qualify for FMLA and OFLA, your doctor must certify that you have a serious health condition and must also state that you are unable to work because of it.

3. Complete Leave Tracking.

  • The FMLA/OFLA leave tracking form needs to be submitted by the 10th of the following month in which you have taken the leave (in addition to regular monthly reporting).
    • EXAMPLE: If you took leave in June, that leave must be reported by July 10th.
