Contact information (address, phone number, emergency contact)

To update your contact information on your student account, you may do so online in the 'My Account' page in myPSU or by downloading, completing, and submitting the Student Information Change Form to the Office of the Registrar either in-person or via your pdx.edu email account.

Race, Ethnicity, Gender, Sexual Orientation

To comply with federal requirements for statistical reporting and affirmative action monitoring, Portland State University must ask for the following demographic information. This information will only be shared with those who have an authorized need to know. We encourage you to confirm or provide this information, but doing so is entirely voluntary and will not affect your student enrollment status or terms or conditions of your employment. The ultimate goal in collecting and analyzing this information is to ensure equal opportunities for everyone in the PSU community.

You may update your race, ethnicity, gender, or sexual orientation via the 'My Account' page in myPSU.

Legal Name change - RO with documentation

If you have legally changed your name, you may update your student record by downloading, completing, and submitting the Student Information Change Form to the Office of the Registrar, either in-person or via you pdx.edu email account. Proof of name change is required. Acceptable documentation includes government-issued ID (driver’s license, passport, etc.), marriage license or divorce decree, or affidavit of legal name change. 

Legal Sex Designation

For Portland State records, your legal sex designation should match what is recorded on a government-issued ID (such as driver’s license, state ID card, or passport), and may be different than your gender. If you are not a citizen or permanent resident of the US, your legal sex designation should match your passport. 

If you wish to change your legal sex designation with the university you may do so by downloading, completing, and submitting the Student Information Change Form to the Office of the Registrar, either in-person or via your pdx.edu email account.

Preferred Name

To add or change your preferred name, submit the View/Update Preferred Name form, located online on the 'My Account' page in myPSU.

If you wish to change your legal name with the university you may do so by downloading, completing, and submitting the Student Information Change Form, along with proof of name change to the Office of the Registrar.

Chosen Name

If you would like to use a different name, in order to affirm your gender identity, but have not made a legal name change, you may submit a Chosen Name Override form to the Office of the Registrar or the QRC. This will ensure that your legal name will not appear alongside your chosen name on class rosters or in other university spaces.

Social Security Number Declaration or Update

Although SSNs are not used as primary student identifiers, the University collects and securely maintains SSNs for several important reasons. Students are asked to voluntarily provide their SSN to assist Portland State University (PSU) (and organizations providing contracted services or research for PSU) in managing student identity and records security among students with the same name; in administering financial aid benefits; in complying with federal tax reporting laws; in satisfying our reporting requirements to the Higher Education Coordinating Commission (HECC) and to state and federal agencies; in conducting ongoing institutional research, program assessment and instructional improvement; in collecting of student debts; and in comparing student educational experiences with subsequent workforce experiences.

You may use the SSN Declaration or Update web form to report, confirm, clarify, or otherwise notify the Portland State University Office of the Registrar of your Social Security Number (SSN) to be associated with your student record. Documentation is required to update the SSN associated with your student record. This must be a government-issued document, such as a copy of your Social Security Card, Individual Tax Identification Number (ITIN) documentation, or a tax transcript that displays your SSN.
