Course Planners (GM-2)

There is one course plan for each of the graduate degree programs. This form is used in conjunction with the Annual Projected Graduate Courses document below to plan out the coursework over two or more years. It should be completed by the student and the advisor in the first term of admission.

Used to ensure you are completing all the steps in preparation for the recital or final project.

Advisory Committee Forms (GM-4)

Used to assemble your graduate committee.
Master of Music Oral Exam FormThis form must be completed by the student’s advisory committee immediately following the oral exam and given to the Graduate Coordinator. The Graduate Coordinator must submit a DARS exception stating that the student passed their Comprehensive Oral Exam before they can graduate.
Master of Music Degree Change FormThe MM Degree Change form is used if a student wishes to change from one degree program to another for example, an MA to an MS or MM. It may also be used to change from a degree outside of the School of Music to a music degree.
Master’s Degree and Graduate Certificate Proposed Transfer Credit (GO-21M)This form is required to request transfer credits be applied to your graduate degree at PSU from another university. Review the document for details and requirements. This form must be submitted to the Graduate School for review.
Master of Arts/Master of Science Project Guidelines & TimelinesThis form describes the steps necessary to plan for and complete the required project (MUS 506) for the degree. 
Graduate PetitionA student may petition the Graduate Council for a waiver of a University graduate academic regulation or degree requirement outside the School of Music such as use of undergraduate credit. Most requirements can be waived from within the School of Music using the GM-3. This form must be submitted to the Graduate School for review.
School of Music & Theater Petition Form (GM-3)Submit this signed form to the School of Music & Theater Graduate Coordinator who will submit a DARS Exception to include the credits.
Overload ApprovalGraduate students must obtain the appropriate approvals below for registration in excess of 16 credits in a term (graduate and undergraduate credits combined). This form must be submitted to the Graduate School for review.
School of Music and Theater, Annual Projected Graduate CoursesProvides the specific terms that graduate courses are being offered over a period of two years. It is used in conjunction with the GM-2 in order for advisors to help students create their degree plan. It should be updated each spring by verifying course offerings with the School Director.
Procedures for the Master of Music Pre-Recital Jury and Oral Exit ExaminationProvides preparation information for the recital and oral exam. It is important for advisors and students to adhere to the instructions in this document in order to ensure that all the proper steps have been followed.
Graduate Master of Arts/Master of Science Written Exit Examination InformationProvides information on times and how to sign up and study for the exam.
Application for Awarding of Master’s Degree, Doctoral Degree, or Graduate CertificateThe Application for Awarding of Master’s or Doctoral Degree is required by the first Friday of the term in which a graduate student wishes to graduate. This form must be submitted to the Graduate School for review.
