Cost of Attendance Increase Form Request to have the Office of Financial Aid increase your estimated cost of attendance for costs resulting from special circumstances that were not included in your original financial aid calculation.
NEW: COVID-19 Financial Aid Appeal for Reconsideration Use this form to request a re-evaluation of your financial aid based on changes in your or your parents' financial status as a result of COVID-19.
Dependency Override Appeal Form Submit this form to be considered independent for financial aid purposes and therefore, exempt from providing parental data on the FAFSA.
Financial Aid Revision Request Form Use this form to request changes to your financial aid for enrollment changes for future terms and increasing and decreasing your Stafford Loans.
Graduate Academic Progress Petition (2019-2020) This petition is for graduate students who have not met the requirement of enrolling in graduate level credits for at least 67% of their total enrollment in the prior academic year. Parent Financial Aid Re-evaluation Request Form: 2020-2021 Academic Year

Use this form to request a re-evaluation of your financial aid based on changes in your parents’ financial status. Learn more about revisions and re-evaluations.
Satisfactory Academic Progress Petition Form If you are in Suspended status, you may submit a petition in writing to the Office of Student Financial Aid & Scholarships which documents special or unforeseen circumstances that interfered with the completion of your coursework or in meeting any other part of the policy. Student Citizenship Documentation Form Submit this form to verify your eligibility for Financial Aid as a U.S. citizen or eligible noncitizen.

Form or Petition Description
Expedite Refund Request Refunds are issued on accounts when there is a credit balance. Normal processing times for student account refunds after initial disbursements are between 4 and 6 weeks. If you have a compelling reason to receive a refund sooner, submit this form.
FERPA (Release of Non-Directory Information) Form A student may fill out this form to request that PSU allow someone else (parent, guardian, employer, etc.) access to your university information. Student Financial Services cannot speak with anyone about your account, including billing information or amount due, without this form on record. Fill it out and return it in-person, with photo ID to the Registrar's Office to grant access to your account.
Late Fee Petition Petition a $100 late fee by providing a brief explanation of any extenuating circumstances that led to your late payment.  
Lost Check Affidavit If a refund check is lost, stolen or destroyed, use this form to report the loss and request that another check be issued. This form cannot be submitted until 14 business days after the check was first issued.
Returned Check Fee Petition Use this petition to appeal both the $25 returned check fee and the check payment restriction that results from having two check payments returned.
Return of Title IV Funds Use this form to return part or all of your student loans within the first 14 days of disbursement to your student account.
Student Records Privacy Request Use this form to request that all of your student information, including directory information, be unavailable to parties other than the student and the school. 
Third Party Agreement (TPA) Application Portland State University will invoice sponsors that require a separate invoice for the items on the student bill which the sponsor will pay in support of a student. To be considered for the TPA billing process, sponsors must complete this application. Learn more about setting up third party payment on behalf of a student.
