Reporting Near Misses

Near misses are incidents that have not resulted in an injury or illness.

It is estimated that for every 300 near misses there may be 29 minor injuries and one (1) major injury, including fatality. 

In order to prevent a future incident or injury, all near misses should be reported to EHS so that any corrective actions can be implemented. 

Complete the Near Miss Report form and submit to EHS. 

Reporting Injuries

For reporting a work-related injury or illness please submit an Injury Report Form with Human Resources (required).

If you are seeking medical care for a work-related injury or illness you will also need to submit the SAIF 201 form to Human Resources.

For injuries that occur to non-PSU employees, including visitors of campus, contractors, and non-employed students, please notify EHS directly of the incident by emailing EHS-group@pdx.edu.

Investigations

EHS conducts incident and near miss investigations, focusing on fact-finding and root cause analysis.  Investigation findings are reviewed by the PSU Safety Committee and the PSU Facilities and Construction Safety Committee to help management identify the root causes of accidents and to determine how to prevent them. 

The incident and near miss analysis process is not meant to assign blame or call out one individual’s failure, but to review the entire process that occurred around the incident and look for areas to improve the process to prevent future incidents.
