CAPS is excited to announce that we have made updates to our PSC, PSI, and Contract Cover Sheet forms, and we have added new training materials to our website based on feedback from the campus community.
Effective Wednesday, January 18th, 2023, please use the updated versions of the PSC, PSI, and Contract Cover Sheet forms which can be found on our "Forms & Resources" page.
If you run into any issues accessing the forms, please reach out to our general inbox at contract@pdx.edu with the subject line of "New Forms and Training Resources".
New PSC Form
We have integrated the cover sheet information into the new PSC Form, which means you no longer need to fill out and complete a separate PSU Contract Cover Sheet when submitting a PSC. In addition to this, we have also made a few other minor changes to the PSC in an effort to streamline and clarify various sections. We have updated the PSC Instructions to reflect these changes.
Foundation Signature Line Added to Forms
As requested by the Foundation (PSUF), we have added PSUF signature lines to the PSC, PSI, and Contract Cover Sheet Forms. Similar to the Sponsored Project signature lines on our forms, the PSUF signature line needs to be signed if you are using PSUF funds to pay for your contract.
