The GO-21M is now used only for Transfer credits (credits taken at another college/university).
Requests for Pre-admission credits taken at PSU are now made using the Graduate DARS Exception Request form (online). Information on DARS is available here.

Thesis and Dissertation Forms
The Survey of Earned Doctorates (SED) is online. Please register and complete the SED at: https://sed.norc.org/survey. Additional information on the online SED is available here (scroll to bottom of page).
Admission Information and Applications
Application to a graduate program at PSU is a two-step process. You must submit two separate applications: one to the Office of Admissions, and the other directly to the department. Click for instructions and a link to the online Office of Admissions application, as well as instructions for departmental applications.
