PSU Insurance Overview

PSU requires all students taking 5 or more in-load, non-Restricted Differential Tuition credits (1 credit for international students) to have comprehensive medical insurance coverage. Listed below are important links to help address the health insurance policy, waiver process, and to help answer any questions.

2016-2017 PacificSource Benefit Highlights

  • Unlimited medical maximum
  • No pre-existing condition limitations
  • $300 in-network annual deductible
  • PacificSource Provider Network
  • $5,000 out of pocket max for In-Network/$10,000 out of pocket max for Out-of-Network
  • Emergency Room Copay $250
  • Hospital and Surgery coverage
  • Outpatient Physician benefits paid at 100% if In-Network with $25 copay
  • Outpatient Counseling visits paid at 100% if In-Network with $25 copay
  • 80% In-Network/50% Out-of-Network Coverage for most other services (may a have separate copay)
  • Diagnostic X-ray/Lab/MRI coverage
  • Physical Therapy and Chiropractic coverage
  • Prescription coverage (includes birth control)
  • Maternity expense
  • No referrals required to see a specialist
  • 80% coverage for Impacted Wisdom Teeth Removal

2016 - 2017 Insurance Cost Per Term

The student health insurance plan for the 2016-2017 academic year is:

  • $827 for each eligible term (i.e. Fall, Winter and Spring/Summer* combination). 
  • 12 months of coverage is $2,481.
    • The per-term amount will automatically be applied to the student's account when they register for classes. In order to have the charge removed, they must complete the insurance waiver (one per academic year).

    2017 - 2018 Insurance Cost Per Term

    The student health insurance plan for the 2017-2018 academic year is:
    • Fall Term $875
    • Winter Term $875
    • Spring/Summer $875
    • Summer Only $631

    Please note: This is a specifc FERPA Release Form that pertains only to the PSU Student Health Insurance Plan and does not include the release of other FERPA protected information or Protected Health Information (PHI). If you would like members of the SHAC Insurance Team to discuss the PSU Student Health Insurance Requirement or waiver process as it specifically pertains to you, a signed copy of this form is required.
