PSU Student Health Insurance Plan

Students WITHOUT Health Insurance: PSU Student Health Insurance Plan 

All students taking five or more credit hours per term (1 credit hour per term for international students) you are required to have comprehensive medical insurance coverage. If you do not have health insurance or your health insurance does not meet the criteria for the waiver application, you will automatically be enrolled in the PSU Student Health Insurance Plan and charged $875 to your student account. The deadline to submit the waiver application for fall term is October 8, 2017 at 11:59pm. 

Eligibility

All students taking five or more credit hours per term at PSU (1 credit hour for international students) are eligible for the PSU Student Health Insurance Plan. All students are required to have health insurance that meets PSU Insurance Plan Criteria. Students who drop below five credits in the first two weeks of the term will no longer be eligible for the student insurance that term and will not be charged for the PSU Student Health Insurance Plan. If a student needs to drop all courses for medical reasons but wishes to continue to have the PSU Student Health Insurance Plan, they may appeal to do so by contacting the insurance team. 

How it Works 

If you are covered by the PSU Student Health Insurance Plan, you will be enrolled automatically and charged $875 (per term). You will be charged this rate on your PSU student account each term, as long as you are taking five or more credit hours AND do not submit the health insurance waiver application.

Please note: This is a specific FERPA Release Form that pertains only to the PSU Student Health Insurance Plan and does not include the release of other FERPA protected information or Protected Health Information (PHI). If you would like members of the SHAC Insurance Team to discuss the PSU Student Health Insurance Requirement or waiver process as it specifically pertains to you, a signed copy of this form is required.
