There are three types of forms on this page:

• Interactive PDF forms, which can be filled out online. With the form open, click the Highlight Fields box, choose the hand symbol tool, and tab through to type your entries. Note that some of the forms now automatically create a duplicate as you type. After filling out the form, print it, and sign and date it where indicated.
• Printable PDF forms, which do not have Highlight Fields and are not interactive. You will need to print it out before completing it.
• Online Forms, which can be filled out and submitted online.

For Medical Withdrawal: When dropping all courses for the term due to medical reasons please submit the Medical Verification Form with your Deadline Appeals Board petition.

Alert to students on financial aid who are dropping all classes: A financial aid counselor must sign the petition before the Deadline Appeals Board will review it.
