Revision Request Form 

Use this form to make changes to your financial aid such as enrollment changes for future terms and many types of student loan changes. 

Parent | Re-evaluation Request 2007 / 2008 (pdf, 84kb)

Use this form to request a re-evaluation of your financial aid based on changes in your parents' financial status. Petitionable circumstances include the following: divorce, separation, or death of a parent whose income is listed on your FAFSA; loss of income due to disabling illness or injury; involuntary loss of employment; involuntary reduction or elimination of monetary benefits from an outside agency/person (such as Social Security benefits or child support).

Petitionable circumstances include the following: divorce, separation, or death of a spouse whose income is listed on your FAFSA; loss of income due to disabling illness or injury; involuntary loss of employment; involuntary reduction or elimination of monetary benefits from an outside agency/person (such as Social Security benefits or child support).

Students may have their estimated cost of attendance increased for costs resulting from special circumstances that were not included in the original financial aid calculation. Examples of these costs are significant medical expenses, child care costs, or costs associated with purchasing a computer. An increase in a student's cost of attendance does not automatically equal an increase in a student's financial aid. If you have remaining eligibility for one of your financial aid programs, you will need to request the financial aid office to increase that specific type of financial aid to cover the additional costs resulting from your request for a budget increase.

Work-Study Change Request

Use this form if you want to reduce or cancel your Work-Study award. It may be possible to replace this money with loan funds.

Parents of the student may print this form and submit it along with a signed copy of their federal taxes to complete their portion of this requirement.

Students | Verification

The Student Verification Worksheet must be accessed through the student information system and be completed online.

Veteran's Educational Benefits Certification (pdf, 38kb)

Submit this form to provide verification of the amount of Veteran's educational benefits you will receive during the academic year.

2007 / 2008 Outside Funding Notification

Use this form to notify our office of any additional aid you are receiving, such as scholarships, graduate assistantships, or tuition reimbursement.


Loans

Lender Form

Use this form to notify us of your lender selection for your Federal Stafford Loans. As part of the award acceptance process you will be given a choice of lenders. If you did not select a lender, or have a lender that wasn't listed as part of that process, please submit this form.

Graduate PLUS Loan Request Form (pdf, 77kb)

Complete this form if you are a Graduate student seeking a Federal PLUS loan. To be eligible for this loan, your cost of attendance must exceed the financial aid offered to you.

Parent Loan (PLUS) Request Form (pdf, 96kb)

Complete this form if you are the parent of a PSU Financial Aid Applicant and are applying for a federal PLUS loan. To be eligible for this loan, the student must be dependent and have unmet need.

Unsubsidized Loan by Exception (pdf, 45kb)

If you are a dependent student whose parents are unable to receive a Federal PLUS loan, fill out this form to see if you qualify for additional Unsubsidized Stafford Loan funds.

Loan Discharged Physician's Certification Form (pdf, 26kb)

Federal regulations require that in order to qualify for additional federal student loans, a borrower who has had prior loans canceled or discharged due to total and permanent disability must provide a signed a statement certified by a physician.

Stafford Master Promissory Note e-Sign Guide

If this is the first time you have applied for a Federal Stafford loan with Portland State University or you are changing your lender, you will need to complete a master promissory note (MPN) for the funds to be disbursed to the school.


Petitions and appeals

Satisfactory Academic Progress Petition (pdf, 34kb)

Submit this petition to appeal your Satisfactory Academic Progress (SAP) suspension status. Please check with the Office of Student Financial Aid if you have questions concerning your SAP status.

Census Date Disbursement Appeal (pdf, 37kb)

If your financial aid for the current term has been reduced as a result of your enrollment at the university census date, you may be eligible to receive all, or part, of this money again, if your registration resulted from a documentable university error. Submit this form along with pertinent documentation within two weeks of the Census Date for the term you are appealing and we will review your request.

Maximum Credit Limit Petition (pdf, 49kb)

A student may receive financial aid for up to 150% of the timeframe required to complete their degree. You can use this form to appeal that limitation.

Dependency Override Petition (pdf, 98kb)

The Free Application for Federal Student Aid (FAFSA) states the criteria used to determine if a student's parent(s) must provide information on the aid application. If the FAFSA instructs you to provide parental information, but you have unusual family circumstances which prevent your parent(s) from completing the form, you may use this petition.

If you need to file a paper FAFSA, you can access that form at the link above. Filing by paper form will delay your application and is not recommended if you are able to file online.

Request for Release of Confidential information (pdf, 93kb)

Confidentiality laws prohibit us from releasing student-specific information to outside agencies or individuals (that includes parents and spouses). If you wish to release your information to anyone besides yourself, either written or verbally, you must fill out and submit this form to the Financial Aid Office.

Academic Progress Status Update

Students can submit this form to indicate that additional grades have been posted to their transcript. This may clear up a Satisfactory Academic Progress problem that prevents aid from disbursing.

GRE Fee Reduction Request Form

Submit this form to request a GRE Fee Reduction. Please consult the GRE guidelines for eligibility before submitting this form.
