When you are able to give 30 days notice for FMLA or OFLA leave, you should provide medical verification before the leave starts. If your need for either FMLA or OFLA leave precludes giving 30 days notice, then you must provide medical verification within 15 days of the employer's request for verification. If the physician's certification is not received within the 15 day time limit, your leave including the use of sick accrual for the absence may be delayed or denied. You may need to provide additional documentation from you doctor in order to keep the University updated regarding your leave status and your fitness to return to work.
Note: You do not need to provide medical verification for parental leave.
