Public School Personnel Reduced Enrollment Fee Privilege Form

This form is used by cooperating professionals to request reduced tuition for Oregon University System courses.

 

Vouchers for Summer 2009 must be received by July 24 at 5pm.  Fall vouchers can be submitted between Sept. 1 and Sept. 23.

 

  1. Read the instructions and FAQ for information about voucher redemption and restrictions.
  2. Fill out the PDF form electronically by clicking and typing in the fields.
  3. Print the completed form and obtain a district approval signature.
  4. Mail the signed form to the address indicated on the form.
