Public School Personnel Reduced Enrollment Fee Privilege Form

This form is used by cooperating professionals to request reduced tuition for Oregon University System courses.

2. Fill out the PDF form electronically by clicking and typing in the fields.
3. Print the completed form and obtain a district approval signature.
4. Mail the signed form to the address indicated on the form.
