Important information about paperwork

Your academic advisor is the faculty advisor you have had since you entered the program. Your internship liaison is the PSU faculty member who completes site visits and assigns grades for your internship. Your site supervisor is your supervisor at your internship location.

Submit to school counseling internship liaison

Copy of your signed internship site information, Form A
Proof of insurance

Submit to your internship liaison at end of each quarter:

Internship Hours Summary Form, initialed by site supervisor, Form F
Internship Quarterly Term Report, Form D, signed by site supervisor

Submit to your internship liaison at end of internship:

Internship Hours Summary Form, initialed by site supervisor, Form F (final quarter)
Final Summary Report, Form E
