Guidelines for Sabbatical Leave Applications

Sabbatical leave (SBLV) applications are due in CLAS Dean’s Office by January 18, 2008.

Instructions:

We recommend using a blank application as a worksheet before completing the official application.

Besides the information asked for on the form itself, you will be asked to provide three other items:

1. A statement of purpose for your sabbatical leave request

This summary statement (1-2 typed, doubled-spaced pages) should discuss the proposed SBLV activities and be informative both to other specialists in the applicant’s discipline as well as to other literate readers. The statement should include:

A. objectives for the proposed leave and expected significance;

B. relation to longer-term goals of the applicant’s professional career;

C. relation to the applicant’s research and/or teaching assignments;

D. a general plan of how the leave time will be spent;

E. a description of any formal or informal study, experiments planned, lectures scheduled, teaching assignments contracted at other institutions, etc.;

F. a description of any grants applied for or received to support the leave.

2. A current Curriculum Vitae

3. Your Department Chair’s statement of support to the Dean that includes:

A. a plan to cover faculty member’s classes during leave;

B. a request for funds needed to cover classes;

C. a justification/support statement for exceptional cases;

D. chair's signature

 

Other helpful information for completing the application

 

Eligible faculty should satisfy the following requirements:

 

1. Rank of: Senior Instructor, Assistant Professor, Associate Professor, Professor, Research Associate, or Senior Research Associate

 

2. @ .5 FTE or more: continuous appointment without interruption by a SBLV--can be interrupted by authorized leaves of absence (not SBLV). A one year appointment at less than .5 FTE will count as a period of accumulated service to meet the time requirement.

 

FOR EITHER: 18 quarters for 9 month faculty (excluding Summer Session),

6 years. (72 mos.) for 12 month faculty

 

Or: Accumulating the equivalent of 6.0 FTE years over an indefinite period

uninterrupted by SBLV.

 

AN AUTHORIZED LEAVE WILL NOT PREJUDICE ELIGIBILTY FOR SBLV.

 

 

  • Use the following dates for all applications:

 

FALL TERM 9/15/2008 TO 12/14/2008

WINTER TERM 12/15/2008 TO 3/15/2009

SPRING TERM 3/16/2009 TO 6/15/2009

 

 

  • Determine yearly salary basis during this period of leave by selecting one of the following:

 

60% salary = 3 terms leave requested

75% salary = 2 terms leave requested

85% salary = 1 term leave requested

 

  • Fiscal year employee, please see Administrative Rule 580-21-230 (listed on back of the official application) to determine length of leave and salary percent.

 

  • Salary rate for the fiscal year is completed by listing all ranks held and corresponding 9 months of service or full-time equivalency (include this year--2007-2008).

 

  • Example: Assistant Professor: 1979-80 through 1984-85
  • Associate Professor: 1985-86 through 1989-90
  • Professor: 1990-91 through present

 

 

  • When forwarding an exceptional case leave based on other than full-time service (in the 6.0 FTE years or 18 quarters counted toward eligibility, attach exhibit showing only eligibility years counted and FTE associated with these years (no need to indicate salary computation). This exhibit is required of all faculty who have not been at 1.0 FTE during each of the eligibility years or quarters of employment at PSU counting toward sabbatical eligibility.

 

  • List dates of previous sabbatical leaves (mm/dd/yyyy to mm/dd/yyyy) or check “none,” if applicable.

 

  • Indicate the number of pages of attachments incorporated into this contract by reference.

 

Example: ( attachment #1: 2 pgs.; attachment #2: 3 pgs.)

 

REMINDER: At the end of each sabbatical leave the recipient must submit a detailed SBLV Report to the Department Chair for forwarding to the Dean; the Dean will forward it to the President (see “Sabbatical Leave Reports 580-021-0215” on the back of the SBLV Application). This report is due no later than the 4th week of the term that the member returns from sabbatical leave.
