Request a practicum by completing this form. Do this one term in advance of when you plan to do your practicum in order to allow time to arrange for a supervisor and placement, if needed.

Practicum application deadlines are:

• June 30 for fall practicum
• October 1 for winter practicum
• January 15 for spring practicum

I understand that before I can be considered for placement I must:

• Submit my resume to the Field Placement Office

I understand that before I can begin my advanced practicum I must:

• Complete all program prerequisites
  • CI 432/532 Computer Applications for the Classroom
  • LIB 433/533 Children’s Literature K-5
