Perkins Loan Deferment forms

If you have questions about your NDS/Perkins Loan, you may call the Portland State University Accounts Receivable Office.

Please note: If you send an email or leave a voicemail message requesting a form you cannot print, you will need to specify which form you need and make sure we have your current mailing address.


Perkins Cancellation forms may be filled out and printed from your computer:

Submitting a printed form:

1) Fill it out

2) Print it

3) Sign it

4) Then have it certified by the appropriate person.

5) The Cancellation Form should then be mailed to:

Each form has information on what it takes to qualify for the cancellation. Be sure to read the form completely before you ask your human resources office, or the appropriate person, to certify that you qualify for the cancellation. If you have questions about your qualification you may call us at one of the above numbers.

No cancellation may be processed based on work the borrower preformed before receiving the loan or during the time she or he was receiving the loan.

File when you start the qualifying work to have the concurrent deferment started. Then file at the end of each year for cancellation based on the year just completed and for deferment for the next year.

Except where noted cancellation rates are:

1st year: 15%, 2nd year: 15%, 3rd year: 20%, 4th year: 20%, 5th year: 30% Cancellations are processed when we receive the certification after the year has been completed.

PLEASE NOTE THAT YOU MUST BE EMPLOYED FULL TIME TO QUALIFY FOR ALL LOAN CANCELLATIONS.


Teacher


Must be teaching in a public or other nonprofit elementary or secondary school system and must be directly employed by the school system.

The Department of Education web page gives information on the Teacher Cancellation benefit and a link to the list of Low Income schools approved for Teacher Cancellation.

We also use this form for full-time staff members in the educational part of a preschool program carried out under the Head Start Act.
(Cancels at 15% per year)

And for providers of early intervention services in a public or nonprofit program under public supervision. Early intervention services are defined as "Those services defined in section 632(4) of the Individuals with Disabilities Education Act that are provided to infants and toddlers with disabilities." Infants and toddlers with disabilities are defined as "Infants and toddlers under age three, inclusive, who need early intervention services for specified reasons, as defined in section 632(5)(A) of the Individuals with Disabilities Education Act."

Law Enforcement or Corrections Officer

Provider of Services to Low-Income High- Risk Children and their Families

NOTE 1:
The Department of Education has determined that an elementary or secondary school system or a hospital is not an eligible employing agency.
NOTE 2:
If your human resource office or supervisor cannot answer an unqualified Yes to all items on the Criteria Check List you DO NOT QUALIFY for this cancellation.

Nurses and Medical Technicians

NOTE:
Not all Americorps volunteers qualify for this cancellation. Read the form carefully to determine if you do.
(Maximum cancellation is 70%)

Military Service

The service must be in an area of hostilities or an area of imminent danger that qualifies for special pay under Section 310 of Title 37 of the U.S. Code.
(Maximum cancellation is 50% of the initial loan principal at 12.5% per year)

Borrower's Total and Permanent Disability

Call us to obtain the form needed for discharge of the loan.

Borrower's Death

We are required to obtain an original or certified copy of the borrower's death certificate to discharge an NDS/Perkins loan based on the death of the borrower. Please send the certificate to the above address or call us and tell us the location of the borrower's death (state, city, and county) so we can order one.
