New Hires: To obtain a faculty/staff ID card you must have a valid government issued Photo ID. Your first PSU Faculty/Staff ID card is issued free of charge.
USGS and OTRADI personnel are granted Special Appointment status and thus require a PSU ID number. All Special Appointment personnel receive Faculty/Staff cards, and require the ID Authorization form from HR.
Replacement IDs: To obtain a replacement Faculty/Staff ID card please come to ID Services with a valid driver license or passport. There is a $15 fee for this service.
Expiration Date Stickers: These stickers will be on your authorization form when you obtain your first ID. Cards without a sticker or stickers that have expired are revoked of all privileges. About a month before your sticker expires, your department head will issue new stickers to all members of your department. You may also obtain a new expiration date sticker at any time from ID Services or Human Resources.
Expiring Cards: All cards have an expiration date. Cards issued prior to December 31, 2004 have expiration limits 8 years or less. Cards issued after January 1, 2005 have 10 year limits on them. Cards expire the day before their issue date. If your card was issued after July 1,2010, The expiration date will be printed on the back. This is the expiration of the card itself. If you have active access permission on your card, ID Services will send an e-mail notification 30 days prior to your cards expiration. If your card is expired, your access will not work even tough you may have valid access or are currently employed with the university.
Building Access: If you need access to an office or building, please contact your department head. They will need the access badge or PROX card number from your Faculty/Staff ID card. ID Services cannot assign access to your card. If you suspect your card is not working, please hold it next to an access card reader and see if it beeps or turns the light green. If it beeps, or flashes green your card is physically working you just don’t have access. Please contact your department head. If it does not beep, your card is faulty. Please come to ID Services with a valid driver license or passport.

Leaving PSU: Please return the card to ID Card Services. If you have a Tri-Met Passport sticker, please return the passport sticker to Transportation & Parking Services first to stop future TriMet charges.
Emeriti: Thank you for your service to Portland State. To obtain an Emeriti ID card, we must see your Emeriti letter signed by PSU President Wim Wievel. There is no charge for initial or replacement Emeriti ID cards. Emeriti card do not require a validation sticker.
